Job Title:  “Create” Coordinator

Location:  Atlanta, GA

Department:  PVG – Atlanta ASC’s

Reports To:  Clinic Manager “lead” for “create”

FLSA Status:  Exempt

Position Summary: In this position you will be responsible for the overall coordination of the “initial access creation” scheduling, communication and record keeping for patients in need of an initial “access procedure” for dialysis. The “create coordinator” will be responsible for ensuring each patient referred for an initial access procedure is closely monitored, communicated with, and “navigated” through the process efficiently (vein mapping, type of access decision, access procedure scheduling, and tracking of critical data such as the maturation of permanent accesses, transpositions, longevity, etc) to ensure PVG delivers a “best in class” program for patients in need of dialysis access and in accordance with current clinical standards. 

Essential Duties and Responsibilities:

  • Receive referrals for initial access creation for all GA ASCs
  • Develop and maintain a detailed inventory of patients in various phases of the initial access creation process
  • Communicating with all stakeholders, including physicians, ASCs, and patients, to ensure optimal efficiency for patients in need of a permanent dialysis access “create” procedure
  • Personal “navigation” of each patient through the comprehensive process in as timely/efficient path as possible, according to benchmarks established by PVG physicians
  • Developing relationships with various stakeholders in the organization to help facilitate the efficient and effective coordination for care of access creation patients
  • Responsible for careful tracking of all relevant data to closely monitor and evaluate the effectiveness of the overall program, including (1) efficiency from referral to create procedure, (2) time to maturation of permanent access, (3) longevity of success/sustainability of permanent accesses, and (4) other relevant information as deemed necessary by PVG physicians
  • Prepare data tracking and outcomes reports to be shared with PVG physicians and referring physicians, quantifying the performance and effectiveness of the overall program.
  • Develops a good working relationship with dialysis units, nephrologists, and other referring agents to assist in coordination of
  • Responsible for requesting and obtaining appropriate information from the referring physician/facility to safely and appropriately schedule and treat patients in the center
  • Responsible for physician(s) hospital schedule including obtaining pre-surgery labs, cardiac clearance, and others as required to schedule patient for surgery at the hospital.
  • Responsible for clearing all patients on the surgery schedule ensuring preauthorization has been obtained if applicable, patient has been appropriately scheduled in AdvancedMD, cardiac clearance obtained, if applicable, patient demographics match the hospital and so on.
  • Ensuring all hospital patients’ medical records have been input into AdvancedMD to support billing.
  • Working with the MD to ensure all hospital billing has been timely completed.
  • Responsible for obtaining copies of all insurance cards and picture ID on new patients and confirming insurance information on existing patients
  • Responsible for assuring referral paperwork is complete on all patients
  • Works with insurance companies to obtain valid prior authorization for services covered
  • Responsible for communicating all information regarding scheduled patients appropriately to the intake coordinators at the appropriate surgery site
  • Manage no shows and reschedules as appropriate
  • Responsible for on-going patient tracking pre surgery and for extended post-surgery, including capture of critical data for clinical study and business metric purposes
  • Responsible for faxing reports as needed to dialysis centers and/or physician’s offices
  • May need to assist the clinical staff in obtaining necessary data for required reports
  • Perform all job functions and employee duties to the highest level of confidentiality ensuring PHI (including electronic) is not compromised.
  • Employees will have access to the applicable PVG network and software solutions relative to the position.
  • Compliance with an adverse incident reporting system.

  • Minimum of 2 years in a medical or office setting required
  • Basic medical terminology and knowledge of medical specialties and insurance is required
  • Minimum two years’ experience in dialysis, interventional radiology, special procedures, cardiac cath lab or general diagnostic preferred
  • Strong computer skills are essential including Microsoft word and Excel spreadsheet
  • Strong organizational skills with the ability to multitask
  • Strong interpersonal skills

Education/Experience:

Minimum of a high school diploma or general education degree (GED); and 12 months related experience and/or training; or equivalent combination of education and experience required. Advanced education is a significant advantage

Computer Skills:

To perform this job successfully, individuals will have knowledge of business office procedures and will have strong computer skills and experience with Electronic Health Record (EHR) and Microsoft Office: Excel, Word and Outlook.

Language Ability:

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write effective routine reports and correspondence. Ability to speak effectively to customers or employees of an organization.

Job Description:

Responsible for gathering and evaluating critical data and supporting the team. Works closely with customers to troubleshoot and correct problems relating to multiple technologies and assists with implementing initiatives across Information Technology (I.T.). Uses operational and accounting expertise to seek out, research, and evaluate various solutions to provide customers with high-quality and efficient solutions. Maintains and supports project assets, communications, and databases. Creates, manages, and distributes project reports. Utilize accounting skills to properly account for information technology as capital vs. expense and applicable annual amortization. Must stay current on any tax credits the company may be eligible for due to enhanced I.T. systems. Responsible for ongoing installation, configuration, and testing to ensure the computer network remains safe against security threats and has strong Cybersecurity.

Job Requirements:

Requires a Master’s degree in Information Systems and Analytics or a directly related field plus two (2) years of healthcare experience.

Requires two (2) years of experience in each of the following (experience may be gained concurrently):

  • Python, Visual Basic, and SQL.
  • IT hardware and software experience, including Windows, office 365, AdvancedMD, and Scripting.
  • Managing VOIP system.
  • Experience in Microsoft Excel Macros.
  • Managing day to day operations of Information systems.
